Advance Healthcare Directives


An advance healthcare directive (AHCD) is a way to make your health care wishes known if you are unable to speak for yourself or if you prefer someone else to speak for you. AHCD's often include information to indicate your preference regarding treatment options, surgical procedures, cardiopulmonary resuscitation and organ donation.

An AHCD can serve one or both of these functions:
- Power of Attorney for Health Care (to appoint an agent to speak on your behalf)
- Instructions for Health Care (to indicate your wishes).
